A Free Attack is an unopposed melee attack on an opponent who has moved into or through a threaten space. The attacker rolls a normal attack and an action. If the character has already performed an action in that second or has no actions remaining in the round, they can perform a free attack. The free attack counts as an action, a character who makes a free attack can perform no other action in that second.
Each character or creature in melee has an area around themselves that is considered a threatened space. The size of the space is dependent on the weapon in use and the size of character wielding it. For man with a small or medium weapon, the threatened space is the squares to either side and the three squares directly in front of the character.

The picture below illustrates with a red border, the spaces that Carter threatens while he faces North. Any creature moving into or through those spaces bounded by red, risk a free attack from Carter.
Thug2, tries to get past Carter, moves into a threatened space and Carter can make a free attack.
